The H&R Block Financial Advisors Health Care Plan provides affordable options for associates and their families. Insurance becomes effective the first of the following month after the start date. Associates may participate in one of the following medical plans.
Medical Insurance
H&R Block Financial Advisors offers a broad, comprehensive protection plan that will fit your personal needs. Your co-pays, deductibles and the services covered all depend on the program you choose — CCP, MAX CCP, HMO, PPO or EPO. Depending on the medical insurance you choose a prescription drug program may also be available for prescription medications.
Dental Insurance
H&R Block's Dental Plan is self-insured and administered by Ameritas Life Insurance Corporation. Associates may choose a PPO provider or may choose a provider not in the network. Preventive and diagnostic services are covered at 100% and other dental services (excluding orthodontia) are covered at 50% to 80%, subject to a deductible and calendar year maximum. Benefits are limited to $1,500 per calendar year per covered individual. Orthodontia is covered at 50% (subject to deductible) for both children and adults, subject to a lifetime maximum of $1,500 per person. Associates may choose a PPO provider or a provider not in the network.
Vision Insurance
Vision care benefits are self-insured and provided through the Vision Service Plan (VSP). Associates may choose a vision care provider from VSP's extensive network or a provider not in the network. Plan benefits provide for one eye exam and one pair of lenses every twelve months and frames every twenty-four months. There is a nominal co-payment for services rendered by network providers. Services provided out-of-network are reimbursed according to VSP's non-member reimbursement schedule.
